DPC is a healthcare payment and service model where patients pay membership fee directly to a primary care provider (PCP) instead of using insurance for routine visits. This model is designed to offer affordable patient-centered care by eliminating insurance-related complexities.

• · Primary care services:
• · $75 initial visit and $50 for follow up visit or sick visit
• Simple, low-cost primary care management virtually in the comfort of your own home or space. Please be advised that this is a cash-based service. As a cash-based service, AJF Medical Services does not accept insurance. Patients are responsible for payment at time of service. A superbill may be provided upon request.

•  Short term medication refills:
•  $50 up to 5 medications
•  If you are out of your chronic medications for conditions such as high blood pressure, diabetes, and thyroid problems and your doctor does not have any availability for weeks. 30 DAY SUPPLY.

• $50 sick visit: If you are not feeling well and do not want to sit in urgent care, or go to the ER, just set up a telehealth visit from the comfort of your home. This visit includes prescriptions and work/school note if applicable. Prescriptions are provided only when clinically appropriate and are not guaranteed. If you think you are experiencing a medical emergency, call 911 or go to the nearest emergency room.
   

•  $75 single individual monthly:  Includes One Comprehensive Visit, Two  sick visits,  Basic Labs, and Unlimited access to provider at all times within the year.
•  $120 Up to Two individuals  monthly: Includes One Comprehensive Visit, Two  sick visits,  Basic Labs, and Unlimited access to provider at all times within the year.

•  Initial consultation fee $150.  Treatment requires a medical evaluation and may require labs. Medication is prescribed only if clinically appropriate. Results vary; no guaranteed weight loss. Labs may be required before starting and during treatment, based on clinical judgment. Not recommended during pregnancy/breastfeeding.
