Financial Policy
Payment Responsibility
AJF Medical Services operates as a self-pay service and does not accept insurance or provide substitute coverage for catastrophic events. All clients must pay for services at the time of booking and prior to the completion of those services.
Additional Fees
- If labs or additional tests are ordered, clients are responsible for paying laboratory fees and AJF Medical Services’ diagnostic analysis fees prior to completing the lab draw.
Payment Methods
We accept the following payment methods:
- Credit Card
- Debit Card
- Zelle
Upon request, we will provide clients with a superbill. However, insurance reimbursements are not permitted or honored for any of our services.
Membership Fees
For clients enrolled in our membership program, fees are billed on a monthly basis. The membership can be canceled at any time; however, clients must provide at least 7 days’ notice before the start of the next billing cycle to process a cancellation.

Missed and Cancellation Appointment Policy
At AJF Medical Services, we value your time and strive to provide the highest level of care. To ensure that all clients have access to appointments in a timely manner, we have established the following policy regarding missed and canceled appointments:
Cancellation Notice Requirements
- New Patient or Initial Visits: A 48-hour notice is required if you are unable to keep your appointment.
- Follow-Up or Routine Visits: A 24-hour notice is required if you are unable to keep your appointment.
Missed Appointment Fees
- If you miss an appointment or fail to provide the required notice, a $30 fee will be charged for the missed appointment.
- This fee will be deducted from the payment made at the time you booked the appointment.
Rescheduling Appointments
- If you need to reschedule, please contact our office within the required notice period. This allows us to offer the available time slot to another client.

Payments & Past Due Accounts Policy
At AJF Medical Services, we aim to ensure transparency and clarity regarding our payment terms and policies. Please review the following guidelines regarding payments and past due accounts.
Payment Terms
- For non-members, full payment is required at the time of booking any service.
- Payment can be made using accepted methods such as credit card, debit card, or Zelle.
DPC Membership Payment Requirements
- For Direct Primary Care (DPC) members, membership fees are billed on a monthly basis.
- If a credit card payment is declined or two consecutive paymentsare missed, the following actions will be taken:
- Your account will be referred to our collection agency.
- AJF Medical Services will automatically cancel your membership.
Resubscribing After Membership Cancellation
- Should you wish to resubscribe after your membership has been canceled, you will be required to pay:
- A $70 enrollment fee in addition to settling any outstanding account balance.
- For individuals who subscribe and cancel membership on two or more occasions, the following applies:
- An enrollment fee of $100 will be required for future resubscriptions.
- AJF Medical Services reserves the right to deny a renewed partnership at its sole discretion in such cases.

- Completed Acute Visits: Refunds are not provided for completed acute visits or non-member visits where medical advice or prescriptions were issued.
- Weight Loss Products: Refunds are not offered for any weight loss products ordered, regardless of whether they are used or unused.
- Exchanges: We do not offer exchanges for services rendered.
Membership Refunds
- For members, refunds are not available for services that have not been utilized or benefits not yet claimed.
